Our free in home consultation will allow us to assess your individual needs and help you choose the level of care that's right for you. If your needs change, your level of care can change with it. You are still in control of healthcare, you just don't have to do it alone.
Medical Advocate
We schedule medical appointments, link you to transportation, coordinate between all service providers (labs, imaging, pharmacy), and assist with any emergency or inpatient discharge planning. We also assist with referral and second opinion scheduling as requested. We will communicate with families both near and far to keep them up to date with all medical updates.
Personal Advocate
We attend all medical appointments in person for several reasons: it improves communication with medical staff ensuring critical details are not missed; it reduces stress and provides emotional support as it helps the patient focus on recovery rather than managing logistics; it enhances safety by allowing one advocate to coordinate between all providers & prevent medical errors; and it reduces financial and legal risk by regular medical bill review.
Inpatient Advocate
Whether a scheduled surgery or a sudden emergency room visit, we're with you either way. We maintain daily contact with your inpatient medical team for seamless discharge planning and follow up with referrals or post op care.
